Does insurance cover chiropractic care?
Many health plans cover chiropractic care, but coverage is often partial. NCCIH, part of the National Institutes of Health, notes that plans may cover chiropractic, but not always completely. The details depend on whether you have Medicare, a private plan through work or the marketplace, or auto insurance after a crash.
The table below shows how the main types of coverage usually work. Always confirm your own plan, because benefits change from year to year.
| Coverage type | What it usually pays for | Common limits | Where to verify |
|---|---|---|---|
| Original Medicare (Part B) | Manual manipulation of the spine to correct a subluxation | No X-rays, massage, acupuncture or maintenance care; 20% coinsurance after the Part B deductible | Medicare.gov |
| Private health plans | Adjustments, often exams and medically necessary X-rays; sometimes rehab | Yearly visit caps, copays, deductibles, referral or prior approval rules | Member services number on your card |
| Texas auto PIP | Medical bills and some lost wages after a crash, for you and your passengers | Policy dollar limit; only if you did not reject PIP in writing | Your auto insurer; Texas Department of Insurance |
| Other driver's liability | Your injuries when another driver is at fault | Their policy limits; the claim can take time | The other driver's insurer |
What does Medicare cover for chiropractic?
Medicare Part B covers one chiropractic service: manual manipulation of the spine to correct a subluxation. A subluxation, in Medicare's terms, is a joint in the spine that is not moving or aligned properly. After you meet the Part B deductible, you generally pay 20% of the Medicare-approved amount.
Medicare.gov states that it does not cover other services or tests a chiropractor orders, including X-rays, massage therapy and acupuncture. The Medicare Benefit Policy Manual adds that once further improvement cannot reasonably be expected and care becomes supportive rather than corrective, it is maintenance therapy and is not covered.
If you have a Medicare Advantage plan, its rules may differ. Call the plan directly.
How do private health plans handle chiropractic?
Private plans vary widely, even within the same insurance company. The most common differences are:
- Visit limits. Many plans allow a set number of chiropractic visits per year.
- Cost sharing. You may owe a copay per visit, or coinsurance after your deductible.
- Referrals and approvals. Some plans, especially HMOs, require a referral or prior approval.
- Which services count. Exams, X-rays, rehab exercise, massage and decompression may each be treated differently.
- Medical necessity. Many plans pay for care that is improving your function, not ongoing wellness visits.
Does car insurance pay for chiropractic care after a wreck in Texas?
It may. The Texas Department of Insurance says all auto policies in Texas include personal injury protection, or PIP, unless you told the company in writing that you did not want it. PIP pays your and your passengers' medical bills, and also lost wages and other nonmedical costs, up to the dollar limit on your policy.
If another driver caused the crash, their liability insurance is responsible for your injuries up to their limits. When those limits are not enough, TDI says your own company may use your PIP, medical payments or uninsured motorist coverage to pay the difference. Frequently asked questions
Medicare Part B covers manual manipulation of the spine by a chiropractor to correct a subluxation, which means a joint in the spine that is not moving properly. After you meet the Part B deductible, you generally pay 20% of the Medicare-approved amount. Medicare does not cover other services a chiropractor orders, such as X-rays, massage or acupuncture, and it does not cover maintenance care once improvement has leveled off.
It depends on the plan. Many private plans cover X-rays when they are medically necessary, though they may apply your deductible. Original Medicare does not pay for X-rays ordered by a chiropractor, even though it covers the adjustment itself. Ask your plan how imaging is handled before your first visit so there are no surprises.
It may. Every Texas auto policy includes personal injury protection unless you rejected it in writing, and PIP pays medical bills and some lost wages for you and your passengers up to your policy limit. If another driver caused the crash, their liability insurance may also pay. The Texas Department of Insurance advises telling your insurer about the accident quickly, since most companies have claim deadlines.
Some plans require a referral from a primary care doctor, and many do not. Health maintenance organization plans are more likely to require one. Check your plan documents or call the member services number on your card and ask directly whether chiropractic visits need a referral or prior approval.
Ask whether chiropractic is covered, how many visits per year are allowed, what your copay or coinsurance is, whether your deductible applies, whether a referral or prior approval is needed, and whether X-rays, rehab exercise, massage or decompression are covered. Write down the date, the name of the person you spoke with and a reference number.
